Overview

Hexarelin is a potent synthetic growth-hormone secretagogue. Relative to ipamorelin it is less selective: it reliably stimulates GH but also stimulates prolactin, ACTH, and cortisol, giving it a stronger pharmacodynamic footprint and a rougher endocrine side-effect profile.

Mechanism

Hexarelin acts via GHSR1a (the ghrelin receptor) at pituitary and hypothalamic sites to drive GH release through PLC-linked calcium signaling. A separate line of work showed that CD36 mediates cardiovascular actions of hexarelin, including coronary perfusion pressure effects, indicating that not all of hexarelin's biology is reducible to classic GHSR endocrinology.

Robust GH release in humans

A placebo-controlled dose-ranging study in healthy men found a substantial, dose-dependent GH rise after 0.5, 1, and 2 mcg/kg IV, with near-maximal response by 1-2 mcg/kg.

Exploratory cardiovascular benefit

Small human cardiovascular work in patients undergoing coronary bypass surgery reported improved cardiac performance after acute hexarelin administration. Preclinical cardioprotection adds animal support, but the human cardiovascular signal remains suggestive.

Dosing notes

Human endocrine dose-ranging data support 0.5-2 mcg/kg IV bolus testing. Pediatric studies cited in the literature used 2 mcg/kg IV for stimulation testing or 60 mcg/kg intranasally three times daily in chronic growth studies. The pediatric intranasal data are not an approved maintenance regimen but are among the clearest long-term human exposure examples.
