Tesamorelin is a synthetic growth hormone-releasing factor analog, FDA-approved as Egrifta SV and Egrifta WR formulations for reduction of excess abdominal fat in HIV-infected adults with lipodystrophy. The two formulations are explicitly not substitutable. It is not a general weight-loss drug.

Mechanism

Tesamorelin acts at the pituitary to stimulate endogenous growth hormone release and raises IGF-1, which is the basis for its effect on visceral adipose tissue rather than generalized weight loss. The FDA label highlights characteristic effects on visceral fat and IGF-1 elevation across both Egrifta SV and Egrifta WR formulations.

Pivotal phase studies showed mean visceral adipose tissue (VAT) changes of approximately -27 cm² and -21 cm² in the active arms versus little or no reduction with placebo at 26 weeks.

Storage

Per FDA label: Egrifta WR is stored at room temperature; the reconstituted vial may be used for 7 consecutive daily doses before discard. Egrifta SV is reconstituted immediately before use and discarded if not used immediately — the reconstituted solution is neither refrigerated nor frozen.
