Overview

Protirelin is synthetic thyrotropin-releasing hormone (TRH). Historically used as a diagnostic pituitary-thyroid challenge agent. Current U.S. public labeling on DailyMed reflects a compounded product, and PubChem notes protirelin is not currently available in an FDA-approved product.

Mechanism

TRH increases release of TSH from the anterior pituitary and also increases prolactin release. After intravenous administration, plasma half-life is about 5 minutes. Diagnostic value is in assessing thyroid function as an adjunct to other tests and in distinguishing pituitary from hypothalamic dysfunction when interpreted alongside the rest of the endocrine workup.

Reported side effects

Mild and brief side effects in about half of patients

Label reports side effects in about half of tested patients — usually mild and brief. Reported reactions include blood-pressure changes, headache, urge to urinate, nausea, flushed sensation, light-headedness, bad taste, abdominal discomfort, and dry mouth.
Rare serious reactions

Rare but possible: more serious hypotension, severe hypertension, convulsions in predisposed patients, or pituitary apoplexy in macroadenoma patients undergoing combined testing.

Dosing notes

Adult diagnostic dose: 500 mcg IV bolus over 15–30 seconds with the patient supine (200–500 mcg range used). Pediatric: 7 mcg/kg up to 500 mcg. Baseline and 30-minute TSH sampling are standard. Repeat testing should generally wait at least 7 days.
